This is a reference format, not legal advice. Wording, required stamp paper value, and notarization steps vary by state and by institution. Always check your specific institution's or employer's requirement first and use their format if one is provided. Confirm final wording with a notary before signing.

When You Need This

Many students and first-time applicants don't have a utility bill, ration card, or bank passbook in their own name yet - these are usually still registered to a parent or another family member. When a form insists on address proof but every document you can produce is in someone else's name, a self-declaration affidavit lets you formally confirm your current residential address, your relationship to the document holder, and that the address is genuinely where you reside.

Frequently Asked Questions

When an application asks for address proof but your utility bill, ration card, or rent agreement is in a parent's, spouse's, or relative's name rather than your own - a self-declaration affidavit lets you confirm your current residential address while explaining the relationship to the document holder.
Usually not by itself. Most institutions want this affidavit submitted alongside a copy of the underlying proof (electricity bill, ration card, etc.) that's in the family member's name, plus proof of your relationship to them. Check the specific requirement of the authority you're applying to.
Commonly accepted documents include an Aadhaar card, electricity or water bill, ration card, rent/lease agreement, or a bank passbook - whichever is available, even if it's in a parent's or relative's name, alongside this affidavit.
This varies by state - commonly Rs. 10, Rs. 20, or Rs. 100 non-judicial stamp paper is used for this type of affidavit, but exact requirements differ by state and by the authority you're submitting to. Check with your notary or the concerned institution before buying stamp paper.
