When You Need This
Some students never had a birth certificate issued, or their date of birth is recorded slightly differently across their school leaving certificate, Aadhaar, and other identity documents. When an exam form, college admission, or job application asks for a document you can't produce, or your DOB doesn't match cleanly across records, the authority will often accept a notarized affidavit declaring your correct date of birth in the meantime - alongside whatever supporting proof you do have.

How to Use This Format
- Buy non-judicial stamp paper of the value required by your notary or institution - this varies by state, so confirm the exact value first.
- Have the affidavit typed or printed on the stamp paper (or attached to it, depending on your notary's process).
- Sign it in front of a Notary Public - do not sign it beforehand.
- Carry whatever DOB-supporting documents you have (school certificate, Aadhaar, hospital record) along with a valid ID proof when visiting the notary.
