When You Need This

If your name is spelled or formatted slightly differently across your own documents — say, your Aadhaar card and your 10th marksheet, or your admit card and your passport — many institutions and departments will ask for a notarized affidavit confirming that both names belong to the same person, before they'll accept the mismatched document.

Note: this is different from a name change affidavit, which is used when you've actually changed your name (for example after marriage) rather than just correcting a spelling discrepancy — see the FAQ below for the distinction.
